The Arc of Texas (The Arc) creates opportunities for all Texans with intellectual and developmental disabilities (IDD) to actively participate in their communities and make the choices that affect their lives. Since our founding in 1950, The Arc at the local, state, and national level has been instrumental in the creation of virtually every program, service, right, and benefit that is now available to over one million Texans with IDD.

Today, The Arc is primarily a public policy and advocacy organization that continues to advocate for including people with IDD in all aspects of society.

Position Summary

The Care Management Coordinator (“the Coordinator”), working under the Education and Outreach Manager of Master Pooled Trust “the Trust”, is primarily responsible for assisting and supporting beneficiaries and their representatives with accessing and navigating benefits and community resources. The Coordinator is a subject matter expert on entitlements, supported decision making and other alternatives, guardianship, vocation resources/benefits and community resources.

The Coordinator is responsible for supporting people with disabilities and their families accessing and navigating benefits and community resources. The Coordinator will primarily interact on the phone and through email with individuals, families, care managers, attorneys, and other interested parties associated with the Trust. This full-time position requires strong communication, people skills, and organizational skills.

Primary Duties and Responsibilities:

Trust Services

  1. Communication
    1. Communicate with individuals, families, and attorneys to assess the current situation of beneficiaries
  2. Special Needs Trust Rules and Regulations
    1. Keep up with current special needs trust-related information from Texas Medicaid and Social Security.
    2. Understand public benefits and the potential impact of distributions on those benefits.
    3. Understand the disbursement operations of the Master Pooled Trust.
  3. Provide presentations as needed on the Master Pooled Trust and other relevant topics.

Care Management Support

  1. Provide and or connect Trust Beneficiaries and their representatives with care management services.
  2. Manage the care management services to ensure that the beneficiary uses their MPT sub-account to improve quality of life.
  3. Communication
    1. Communicate and develop relationships with beneficiaries, guardians, family members, and attorney(s) to meet the beneficiary’s individual needs.
    2. Communicate with other team members regarding beneficiaries and their needs.
    3. Regularly communicate with Care Managers.
  4. Public Benefits
    1. Helping individuals understand their public benefits and the potential impact of distributions on those benefits.
    2. Make sure spending complies with public/government benefit criteria.
    3. Assist Beneficiaries and their representatives to complete public benefit applications.
    4. Provide regular reports to keep public benefits eligibility and respond to requests from these programs.
  5. Care Management
    1. Identify and coordinate with care managers across the state.
    2. Evaluate and determine appropriate plans for assigned beneficiaries.
    3. Provide basic care management services for beneficiaries as needed.

Systems Development, Maintenance & Improvement

  1. Works with the Education and Outreach Manager to develop and improves systems to facilitate the care management program of the Trust.
  2. Work with Trust team to update Policies & Procedures, FAQ’s, etc.

Special Projects as assigned

Qualifications for the Care Management Coordinator:

  • Capable of working both independently and as a member of a collaborative team, as well as communicating with individuals at all levels of The Arc and MPT.
  • Ability to effectively communicate via phone and email ensuring that all duties are completed accurately and delivered with high quality and in a timely manner.
  • Experience using technology, services, and software like Google Meet, Zoom, Microsoft 365, Box.com and basic CRM databases.
  • Adapts well to new and evolving procedures as we improve and streamline our services and comply with ever-changing regulations.
  • Understand how to be sensitive and tactful when interacting with people who have disabilities and their families.

Educational and Professional Requirements:

  • Minimum of a high school diploma, and bachelor’s degree preferred.
  • Social Work degree or equivalent work experience.
  • At least three (3) years of professional experience; years of experience may be considered in lieu of a degree.
  • Prior work with individuals and families with disabilities is required.
  • Knowledge of disability benefits (Social Security, Medicare, Medicaid, etc.) is required.
  • Spanish Bilingual preferred.

Working Conditions:

  • Approximately 22 co-workers in relatively close quarters.
  • Weekend and evening hours required.
  • Approximately 60% of this job can be performed remotely, with approximately 10% occurring in-office, and 30% in community-based settings (e.g., off-site meetings, etc.).

Compensation and Benefits:

The Arc values its employees, and as such, strives to offer a competitive total compensation package which includes both cash compensation and benefits.

Salary Range: starting at $20.00 per hour ($41,600 annually)
